Because space is limited, please complete & clip this application & mail it along with an $80.00 NON-REFUNDABLE CONFIRMATION DEPOSIT by JULY 7th.  If you register after the set deadline, please add a $20.00 late fee.

To complete the registration process, a parent or guardian must sign the consent form and have the medical form completed prior to the start of your summer training. Your physician's signature is required on the medical form.  All information given is confidential.
