Because space is limited, please complete the application & mail it along with an $80.00 NON-REFUNDABLE CONFIRMATION DEPOSIT by June 14th.  If you register after the set deadline, please add a $20.00 late fee. Tuesdays July 1, 8, 15, 22, 29, and August 5th $200

Thursdays July 3, 10, 17, 24, 31, and August 7th $200

If  5-9 team members attend:
Each will save 5% off published tuition rates!

If 10 or more team members attend:
Each will save 10% off published tuition rates!

To complete the registration process, a parent or guardian must sign the consent form and have the medical form completed prior to the start of your summer training. Your physician's signature is required on the medical form.  All information given is confidential. All individuals attending the club must adhere to and follow all club policies, safety procedures and sign a behavioral contract.  Failure to do so will result in disciplinary action and/or expulsion.
