it along with an $80.00 NON-REFUNDABLE CONFIRMATION DEPOSIT by June
14th. If you register after the set deadline, please
add a $20.00 late fee. Payments are due July 1st unless

To complete the registration process, a parent or guardian must sign
the consent form and have the medical form completed prior to the
start of your summer training. Your physician's signature is
required on the medical form. All
information given is confidential. All individuals attending the
club must adhere to and follow all club policies, safety procedures
and sign a behavioral contract. Failure
to do so will result in disciplinary action and/or expulsion. Any
